ELGIN ATHLETICS REQUIRED PARTICIPATION FORMS

*All Student Athletes wanting to participate in Competitive Athletics at Elgin Public Schools must have the following forms completed and on file with the Athletic Department before the athlete is allowed to practice or compete:

1) Current Athletic Physical (cannot be dated prior to May 1st)
   *Athlete will turn in completed physical to Head Coach or Athletic Department.
2) Medical Release (completed through online forms process)
3) OSSAA Eligibility Record Form (completed online – All 7th/9th Graders and All New Students)
4) Concussion and Head Injury Acknowledgement Form (completed online)
5) Sudden Cardiac Arrest (completed online)
6) Amateurism Awareness
    HB 2615 Oklahoma Statute Section 822.1 Title 70 (completed online)     NCAA Amateurism Guidelines (completed online)
7) OSSAA verification of Rule 9 (No Recruiting Policy) - (completed online)
8) Random Student Drug/Alcohol Testing Acknowledgement – (completed online)

ONLINE FORMS INSTRUCTIONS:

The Elgin Public Schools Athletic Department would like to thank you for taking part in our process of accepting required athletic participation paperwork online.

* All Elgin Public Schools athletes are required to complete the online forms EACH YEAR before being permitted to participate. Parents/Guardians will ONLY have to create a “Parent Account” once. The account will rollover each year until the athlete graduates.

NOTE: YOU MUST HAVE YOUR STUDENT ATHLETE’S STUDENT ID# BEFORE STARTING ONLINE PROCESS

*Information concerning the required participation forms is provided in the information/policies tab.

1. To submit online, you will be asked to provide information such as student athlete’s ID #, student athlete’s full given name, email address, date of birth, year in school, home phone, cell phone numbers, primary physician, emergency contact numbers and other pertinent information. Please make sure you have this with you when filling out your forms online or you will not be able to submit forms.

2. Remember to submit online forms, you and your student athlete must fill out and sign in all required spaces. Forms will not be accepted electronically without everything filled out by the student athlete and the parent/guardian.

3. To access the required athletic participation forms from Elgin Public Schools, hold your cursor over the “ONLINE FOMRS” tab. You will get a drop down list of required forms.

4. TO BEGIN, click on the form name and fill out the information requested. You must have your Student ID# available. This is a requirement for filling out each form. If a question is not applicable, please answer the question with NA or UNKNOWN.

5. TO SIGN THE DOCUMENT, click inside the signature box and hold your mouse down, this will allow you to create an “electronic signature.” If you make a mistake and need to start over click on the refresh icon next to the signature box. If you are using an IPAD, you may create an electronic signature by holding your finger to the screen and signing or by using a stylus pen and signing.
